Joanne Hughes

BSc (Hons)

Trainer & Healthcare Harm Specialist

Joanne’s daughter Jasmine died in February 2011 following failures in her care.

She set up Mother’s Instinct in 2013 as a source of emotional and practical peer support for families whose children die as a result of medical error, and to provide a platform to share their stories and the learning from them nationally to improve patient safety for children.

Joanne is passionate that patient engagement is key to patient safety improvement. She has worked in partnership with the NHS for many years on patient safety projects, in research, and in working groups, that are focused on improving paediatric patient safety to avoid incidents, and improving processes after incidents, particularly the involvement of patients and families in investigations.

Prior to becoming dedicated to patient safety improvement and care of avoidably bereaved parents, Joanne worked as a Senior Medical Representative for Wyeth Pharmaceuticals. She holds a Bachelors of Science in Business Studies and Health Science from the Open University.
