Course objectives

This course will:

  • Clarify the purpose and value of After Action Reviews (AARs) as a key learning response tool within systems-based patient safety practice.

  • Build confidence in facilitating AARs that support psychologically safe conversations and promote reflection, shared understanding, and mutual learning

  • Equip participants with a structured AAR process and practical facilitation techniques, including effective questioning and managing group dynamics.

  • Demonstrate how to explore work-as-done and system influences rather than focusing on individual blame, consistent with restorative and systems thinking principles.

  • Provide hands-on practice in applying AARs to real-world scenarios, including routine work, near misses, and safety events.

  • Show how to capture learning and translate insights into meaningful, proportionate improvement actions that drive safer care.
